How to Retrieve Your AMS Username

If you forgot your username, you can retrieve it by clicking the "Forgot?" link above the AMS Username field on the AMS Credentials Mobile Login page

Enter your personal information (i.e., last name, date of birth, and the last four digits of social security number) and click the "Next" button.

Note: reCaptcha is used to prevent brute force attacks on public facing websites. In AMS, the objective of reCaptcha is to verify the user is a human and not an automated system. If suspicious activity is detected, then the user will be required to respond to an image challenge by making selections.

Note: Click the "Don't have SSN?" link for assistance if you do not have an SSN; and click the "Don't know HHSID" link for assistance in determining your HHSID.

Your AMS username will be displayed on the next screen.
